Volunteer Information Form

Volunteers are required to submit their information each season to assist the association

in linking the completion of certifications, CRCs and other volunteer obligations.

Managers must ensure all Certifications & Courses listed below have been obtained. There are common Forms & Documents available for download as well as Additional Resource links. For other various information please review the FAQs. If you would like something added to the FAQs please contact your Division Coordinator to have it added.

Criminal Record Check (CRC)

All coaches and team volunteers who interact with our players must submit to a criminal record check. Find out more by clicking here.

Respect In Sport (RIS)

Respect In Sport For Activity Leaders is required for Coaches.

Complete the online RIS-Activity Leaders Course. This course must be renewed every 5 seasons. You MUST ensure that you use the pre-registration code and ensure that Surrey Falcons (Surrey Female HA) is selected from the organizations list. If you have taken this course for another sport, please click the link above, log-in and you will find information on how to import your certificate to your Hockey Canada profile.

Concussion Awareness Training Tool (CATT)

The Concussion Awareness Training Tool (CATT Coach) is a required certification for all Team Officials to have completed before participation in any practice or games.

Please note that you cannot go on the ice/bench or be rostered and will not have access to Spordle prior to completing the above three requirements.

On-Ice Helpers (helping on the ice for practices only) need to complete the above three requirements prior to participation. They must also be rostered on your official Hockey Canada roster.
